What to expect at your pain management appointment

If your child’s insurance requires him/her to have a referral to see a specialist, your child’s PCP will need to send a referral to our clinic. You will receive a phone call to schedule your child’s first visit.

The office is open from 7:30 a.m. - 4 p.m. Monday - Friday.

A parent/legal guardian and the child must be at the appointment. Children under the age of 18 may not be seen without a parent/legal guardian.

• Insurance card, if applicable

• Driver's license of parent/legal guardian

• Co-payment

• Your child's pediatrician or primary care physician contact information

• Your child's pharmacy name, address and telephone number

• Any films or imaging previously completed (if not already sent to the clinic)

Yes, after every visit we send a clinic letter to your child's primary care physician, detailing the appointment and recommendations for treatment and follow-up.

As a courtesy to our staff and other patients, please notify us as soon as possible at 214-456-8131 if you need to cancel or change your appointment.

Please call 214-456-8131 during business hours (7:30 a.m. – 4 p.m.) or send a message through MyChart and your prescription will be sent to your pharmacy if appropriate. Please allow 1-2 business days for someone to contact you about refilling the medication. Please be aware that if you have not been seen in our clinic in an extended period of time, you may need to schedule an appointment to be seen before refills will be ordered.
